What Is Breast Explant Surgery?

Breast explant surgery, sometimes called breast implant removal surgery, is a cosmetic procedure that removes a patient’s silicone or saline breast implants. Unlike breast revision surgery, which may involve replacing old implants with new ones, breast explant surgery explicitly removes the implants without the aim of replacing them.

What Are My Options for Breast Explant Surgery?

The goal of breast explant surgery is always to safely remove the breast implants and breast capsule or breast pocket (pocket of scar tissue surrounding the implants). The technique used will vary depending on the condition of your breast implants, the type of implant, and your goals for the procedure. Dr. Bedolla will choose the option most suitable for you, although it may change slightly during surgery, as some aspects of breast quality are only visible once the surgery has begun. The two main techniques are as follows:

Total Capsulectomy

During a total capsulectomy, Dr. Bedolla will remove the breast implant first and then remove the capsule in pieces. This technique is more common with implants that have ruptured, have capsular contracture, or other issues. The capsule may have to be removed from the chest wall and surrounding tissues, as they may become fused.

En Bloc Capsulectomy

Dr. Bedolla does not typically recommend an en bloc capsulectomy. This is a procedure reserved for cancer operations that aims at removing all cancerous tissue at the expense of removing normal tissue, including cartilage and bony tissue. Most surgeons offering en bloc resection do not, in fact, perform an en bloc resection. Dr. Bedolla aims to remove all of the capsule and is successful at doing so in 98% of patients. For those with a small amount of capsule either stuck to the chest wall or other sensitive structures, the remaining capsule is fulgurated (burned off), essentially eliminating any of the remaining capsule. Be assured that no diseased capsule will remain in place after the procedure.

What Procedures Can Be Combined With Breast Explant Surgery?

Breast Lift Surgery (Mastopexy)

Aging, pregnancy, and even the weight of large breast implants can all cause the breasts to sag, in some cases necessitating a breast lift along with the explant surgery for an improvement of the breast aesthetic. A breast lift treats breast ptosis by removing excess skin and lifting the tissues to a more appealing position on the chest wall. Dr. Bedolla commonly combines explant surgery with mastopexy for a complete rejuvenation of the breasts without the use of implants.

What Is Breast Explant Surgery Recovery Like?

Following your explant surgery, your breasts will be wrapped in surgical gauze, and you’ll be provided with a surgical bra to support your breasts as they heal. Immediately after your procedure, expect some swelling, bruising, and slight discomfort—Dr. Bedolla can prescribe pain medication to aid in your recovery. 

For the first few weeks, avoid strenuous activities, especially those that engage your upper body. Get as much rest as possible and follow the recovery instructions provided by our office. Keep in mind that combining your explant surgery with another procedure, such as a breast lift, may extend your recovery period.
